Association Name should be the officially-registered name of the association
Mailing address should be the primary postal address used by the association
Board Members 1-3 are only for external points-of-contact -- their titles are not needed
Daytime Contact is someone who is available during the day in case of emergency or weather cancelations
Governors are those permitted to vote on behalf of the association at Board of Governors Meetings
-- President should be listed first, as labled
-- Up to 4 others are permitted and should be listed in the order of voting priority
Assistant Coaches only need to be listed once (regardless of how many rosters they are on)
